You need to submit a support ticket stating when it happened and how much the insurance cost was. Including which bug caused it is probably a good idea. You'll need to be really patient though. I had to wait 5 weeks and when they looked at my ticket they took money out of my balance instead of refunding it even though they said it was a bug and it needed to be refunded. They then just put my balance roughly back to what it was originally without the refund. I'm on round 3 now and hoping (although not expecting) to get it sorted this time.
