well i can confirm you just respawn until you complete the op but i thought i noticed that during the op if i did die & respawn those are the ops i got no reward for when completed
as there is no rebuy ship cost either i just assumed that was the penalty for getting killed

I've gotten rewards for ops that I respawned in, and I've gotten no rewards for some ops that i did without any respawning. I experienced just now a new error code "Bernstein Panther", which is, I guess, when the carrier you started the operation from, is currently in the jump session
You lose nothing. You just start again at the last check point. Sometimes that is the runner.
You don't have a rebuy.
The not getting paid part seems to be random as stated.
I know this isn't how you meant it, but if one wants to optimize Operations to get the rewards as fast as possible, then death is indeed a good option.
You can either fight your way back to your ship through absolute hordes of NPCs - or simply get killed and respawn at your ship in an instant.

as I understand it you one time have to "release" the Blueprint for Merc-Coins, after that you for each Grade 1-5 need Merc-Coins again to pimp the base BP
