How does the Codex work?

I've visited Jackson's Lighthouse numerous times (FSS and all), yet my Codex will not mark it as Confirmed.

Do I have to visit a different neutron star system for it to do so? I thought visiting the reported locations counted as Codex discoveries.

Sometimes it doesn't, sometimes you have to wait or relog, sometimes it removes stuff you thought you had already done. In theory you just need to visit any neutron star and it should record it as a codex find in the region, but it is....unreliable I think is the word to use.
 
A good idea but really badly implemented? Is how I would explain it. But it has been like this for many a year and we just accept it .
 
Quite possibly the codex just doesn't allow confirmations inside the bubble in the pre-discovered systems - many of my confirmations for really common stars like M and K are outside the bubble where I'm pretty sure I would have come across them inside. Mainly the codex is saying to you 'you need to get out more' :D

Also - I never noticed the 'records' sections for stellar objects - neat!

In which case it's still wildly inconsistent...
In particular I scanned various Thargoids with the xeno scanner; nothing never acknowledged by the Codex.
Scan them with the SRV; that works...
