I understand the whole keepers-filling-enrichment-items-first. (And well-trained, workzones, keeper huts close at hand, etc. etc.) That is not what this is about.
In certain habitats, the keepers are constantly not providing enough food. This happens in very large habitats - I like having one big habitat with the Serengeti animals that benefit from being housed together - but also in smaller exhibits; my current save (the Myer's casino in career mode) it happens with the mandrills exhibit. The keepers are feeding as they should be, except it's not enough for all the hungry (and eventually, starving) animals. I can follow them around, and they are definitely putting out lots of food, but it's gone before all the hungry animals are fed, and then I have to wait until they get done cleaning to be able to summon another keeper to feed again. Thus even though I have about 1.5 keepers per habitat, all well trained, and keeper huts close to everything, I am consistently losing animals to starvation... but only in certain habitats. All the others are behaving normally.
