It wasn't in ED 1.0. To be honest it's a fair point though, the fourth Elite rank may have been in the planning stage before the game was released to players.

The Dark Wheel wouldn't even talk to you til you reached Elite, and previously they were in the one system where the permit lock was to be Elite.
Then things changed, and now The Dark Wheel faction in SD are no longer the REAL Dark Wheel...
Maybe Raxxla was intended to be in SD, but they forgot to model it until it was too late, and now they decided to move it and TDW elsewhere.
Maybe I'm talking out of my backside too, so take it all with a few kilos of salt.

Hacker-types look for changes in the files, and have not found a reference to these mysteries (if they had I'm confident the news would quickly propagate). I wouldn't call it stupid, you just described the whole development process of ED in a nutshell. Look for how many years we had this "stupid" placeholder of the ADS in the game until it finally was replaced with something a bit more meaty. But we needed that placeholder to helped the game get going. Raxxla on the other hand isn't fundamentally needed to play the game and it obviously would be extremely daft to provide a placeholder for it. But indeed, Raxxla could be easily hidden in the Cone sector for instance (or any other permit locked region). ED requires a very long wind, both from FDev and us players. Players who followed the game from the beginning and still are here should have grasped it by now.
I've just tagged 'Raxxla' (it wasn't) in Open, with a whole bunch of other Cmdrs in the system. Not because of how long it took me to scan the system or even how long I was in the system for at all (I was roaming in my SRV for at least an hour or two) but simply because I was in a high jump range ship and I sold the data at the port in a nearby (~1,300ly away) nebula. Having an ADS on board might have saved me a minute or so finding the gas giant but that time simply wasn't a factor.

There was no need to remove the ADS, it offers no advantage not countered simply by it occupying a slot. That others do not like it & want an alternative is fine, but there was no need to remove the old modules.
