What the Codex is going on?!

I saw in the Codex that there are two ‘Reported’ Q-type anomalies in HIP 15310, so off I went. When I got there, there were two Notable Stellar Phenomena in the Navigation panel, but upon investigation I discovered that neither of them were Q-types. Both of them consisted of a barely-visible grey cloud with a few metallic crystals floating about. When I checked the Codex again, I saw that I now have ‘Confirmed’ status for one metallic crystals and one proto-LaGrange cloud – but in both cases, these same phenomena are listed as having been Reported in another system about 450 light years away!

What’s going on? How can I Confirm a phenomena in one system that was Reported in a completely different system? And why is the Codex saying that there are two Reported Q-types in 15310 when there don't appear to be any there?

Reported and confirmed statuses are by sector, not system, but have the system in which they were spotted attached.

A metallic crystal legrange clouds were reported as present in the inner orion spur with someone reporting spotting one in some other system. You confirmed that yes, there are metallic crystal legrange clouds in the inner orion spur, but the one you found was in HIP 15310.

I have no answer for why there was no Q-type.

I passed through HIP 15310 as I was in the area, and all I found were two sets of spiky thingies. I could not locate a Lagrange cloud and moved on.

It's all very odd and, I'm afraid, is just confirming to me that I dislike the FSS a lot !
