45fps with 980Ti and i7-3770K (not oc'd) on 1.5 super sampling - SMAA and VR High - is this normal?

45 FPS is just the vsync working. You might actually be close to 90. I had the same thing, turned a couple of things down a notch (can't remember what it was, sorry) and it jumped to constant 90.
 
I suspect that it's not vsync, but in fact reprojection. If you miss frames SteamVR halves the frame rate and "fills in" every other frame with a faked one to attempt some semblance of smooth play. Turn off reprojection in the SteamVR settings (performance section) to see what your true frame-rate is - it'll probably be closer to 80 I reckon.

I totally don't recommend leaving reprojection off though - you'll start feeling really sick. Instead just use it to see how far off 90 you are, mess with AA and SS, and then turn it back on.
 
Got a 980Ti and a CV1. No way I can push supersampling up. I get the feeling VR High is a futureproofed setting, VR Low I presume is designed for their min spec (980) - so maybe some settings from there can be pushed up a little if you have a Ti?

So I've been tweaking settings between VR Low and VR High... Ambient occlusion and shadow quality seem to have big bearing on FPS.

Even on VR Low though, I can't get entirely hitch-free performance close to suns or planets, which is a bit annoying...
