Advanced Discovery Scanner.

If you have both the advanced scanner and discovery scanner, do you need to still be within range of certain bodies for the discovery scanner to do its job?

The advanced Scanner REPLACES the Discovery Scanner.
They are of the same type, just with varying range.

The scanner you can install as an addition to Discovery or Advanced Scanner is the "Detailed Surface Scanner" This works in conjunction with the other one, does not need to be activated separately and increases the detail / cash return of your scans.

Iirc Discovery Scanner has 500LS range, Intermediate Scanner has 1000 LS range and Advanced Scanner has system wide (infinite) range.

No. The DISCOVERY SCANNER, as the name implies, only finds/discovers planets. The better the scanner, the better the range. It does not improve the details of the planet/body, but it lets you find them more easily.
The SURFACE SCANNER gives you more details about the planet/body, and thus increased the payout/value of per planet/body scan.

I have scanned some planets with the basic scanner only; if I return later and scan again with the detailed surface scanner fitted too will I pick up the bonus?
(Reason I ask is I am wondering if it is worth getting the detailed surface scanner until I have enough for the intermediate scanner.)

You should get some bonus if you use a DSS on something you only did a basic scan on.
 
How do you determine which bodies are "worth it"?
I've so far been unable to see the difference between an ice planet and a different kind in the system map.
