Breach mission unable to complete

I have a mission to breach a mining complex , get to the appropriate data port , upload the virus .. wait until it completes but the quest doesn't update.
Tried to upload again but it has gone from the inventory .. So now the quest has to be abandoned its a waste of time and basic game functionality shouldn't still be this buggy after how many years .

Checked the mission description? Could be that it is a covert mission and you triggered the settlements alarm? In this case mission fails even when you manage to upload the data.

One year. Not plural. During which there have been a bunch of other issues to fix as well.

But yeah, it does happen from time to time. Usually when it occurs you can tell something about the settlement "isn't quite right", like it will have multiple panels to disable alarms or some screens/machines will look like they're unpowered even when the settlement is fully active.
