Even if my ship is perfectly still, limpets expire after leaving the cargo bay and going out pretty often when mining. I have watched them die with external cam without anything interfering with them. right outside the hatch.
Every time i use limpets to collect materials at an encoded or decraded site , the first limpet always dies after returning the fetched article, if my ship is still , or moving ever so slightly forward , if my hatch is pointing away or towards the targets, same results, first limpet dies then the second and third does the job (this on PS4 and on AspX and Phantom.) sometimes the second dies to at once but thats a tad more rarely.
I just got into Deep Core Mining and experiencing a similar behaviour. In many cases, the Collector Limpets do their job just fine, but in other cases they "expire" for no obvious reasons. So I googled for it and found this thread and started to follow the advises I've read here. But still, it is like 3 out of 10 limpets just die without obvious reason.
I target a fragment, the path between the fragment and my ship is clear, there is no obstacle in the way, the distance is between 100-300m, I stand perfectly still, facing towards the target, shoot the limpet and wait. The limpet collects the fragment, returns, dives below the ship and then either puts the load into the cargo hold or I get "Limpet expired" without it collecting the fragment. And the fragment is also gone. It is really weird.
And then - before coming here - I already saw limpets skimming over an asteroid fragment to reach the target without colliding and returning the cargo successfully. So it seems the AI is indeed capable of a thorough navigation and avoiding collisions. So it makes it even more weird they seem to just die randomly.
Edit: Just mining again. And another thing I experience is that when a limpet collected a fragment and returned to the ship, it says "Cargo collected", then "Limpet expired", but I get no additional fragment in my cargo hold. Before: 13 fragments. "Cargo collected" -> "Limpet expired" -> Still 13 fragments ... ?! Happened three times already.
