We have just confirmed that by using a single "vent bomber" CMDR it is possible to prevent instances in Titan space from breaking. What appears to break them is having more than one CMDR hit vents (even if it's not the same vent.) Designate a single vent bomber (person delivering the nanite torpedoes to the vents) per instance as a workaround for these bugs!! And glory to Mankind!
