Died from over-heating

At your insurance companies request, our technicians have reviewed the above report with some concern since it concludes that the absence of a failsafe system was a contributing cause of the loss of this vessel. In point of fact, all of our ships are fitted with such a failsafe post-manufacture. We have subcontracted the fitting of this system to your own Pilot's Federation. The most critical component of it requires the installation of a suitable pilot in the cockpit. We are aware that the Pilot's Federation is currently experiencing a shortage of units matching our exacting standards and has been forced to resort to substituting "compatible" units of a somewhat lesser standard. While under most flight regimes this would be entirely sufficient, it has proved inadequate under extreme circumstances. While retrofitting of vessels currently in use is not an option, we have recommended to the Pilot's Federation that they permanently affix a bright yellow warning label to the inside of the center of the cockpit glass on all vessels that have been fitted with "compatible" pilots rather than original-spec units. These are to be printed in a font resembling the scrawling of black crayon the words "Hey! Don't Screw Up!"
