I have done this many times, got caught out being complacent.
I was leaving a planet. boosted. turned off FA so it would continue on its own while I did something else ( make a brew ). while I was away I heard my ship. BANG it had nose dived on its own and rammed the planet so hard my A+ 3 SB's shields disintegrated. lucky for me I had military grade hull and hull reinforcement package. bounced of planet with 12% hull left. I had to return to base, red-faced due to me only just leaving. then on my way back I saw a NPC do the same twice. lol that cheered me up. its not just me to make a boob

I forgot to set power management and accidental deployed my weapons while taking off, which shut everything down and I rear-ended a planet before I could get everything back online LOL Half way out to Jaques when I done that, was left with 68% hull

My Docking computer once started a Death Spiral at Farseer Base.
It tried to make a tight turn and Boosted on my Cutter. That thing turns on a dime, if that dime was the size of the known universe that is.
Crashed sideways straight into her tower ... the one that stretches up into the sky.
Had 2500mj shields back then and they absorbed all of the impact.
I have twice that today but won't make that same mistake.
