Shouldn't the camera be kept fixed to the chair... i.e. as the chair moves the camera should move with the chair (unless of course they've re-written all the OR head tracking!).

You're right. If you look closely, when the chair is off center, so is his avatar, even though he's sitting in the chair centered.
