Engineer Sarge Moon Missing

I was returning to Sarge to deliver the 15 remaining scan data I need to unlock him, and the moon his base is on in Beta-3 Tucani is completely missing. Check out the screenshot here (Xbox): Image. You can see the A and C moon but B isn't there. Doesn't appear in the nav panel either nor his base, "The Beach".

What's going on? Anyone ever have this happen?

OK nevermind. After flying out to the planet it showed up as if I had never discovered it. Extremely strange because I visited the Beach twice in the past. The game forgot I had been there.
