I have done this.

Usually it's at launch when I've either not checked my throttle is zeroed, accidentally switched off rotation correction or possibly both.

One of the beta's my pitch and roll started working backwards after an update...what an amazing incident within the space station that lift off...I wish I had captured that take off!

I raised my landing gear and ment to hit caps lock for my voice comms in teamspeak, and hit tab instead. I boosted. It didn't end well.
