No you should not as the small print on that form states its Not the september complete transfer..
1 because its not September yet... so thee form is not the complete transfer
2 the form states that it will only transfer 2 content types, credit and cosmetics...

The rest of the FAQ that the actual link was put in clearly stated the form was only to be used buy those who wanted the new cosmetics transfer from and old account on console moved to the PC account they're already actively using on PC as they allready purchased the PC version and using it...

The real reply was...
Commander profiles from that time will no longer be available, so we are unable to restore the save data for them. However, we can help with copying your console accounts' Commander and ship "customisation items" to PC.  You can Submit a Request for this here.

Yikes this might confuse someone terribly.

This is just a wealth transfer request, with a slight change from the old process (temporarily maybe to include cosmetics) where they are not deleting your stuff but this is not a copy of your account with a free Horizons game to PC.
