It can happen if you get the data for a system but then don't hand it in and then lose your ship.

Make sure you hand the data in after you visit her. Just drop into the nav beacon, scan the beacon and you'll be set. Or fly to that planet.

I'm pretty sure Domino's been lacing Hero's protein shakes with Push. Anything might happen.

Don't believe me? Visit Hero. Watch her talk with her hands. Then watch her talk with her hands when she isn't saying anything.

there is no module needed to scan beacons, right?

im in my explorer conda ...
and im not sure if i removed something from it to get high jump range

it should just be point the ship at the beacon and it scans, right?

Target it. It passive scans it automatically.

You do have to drop into the nav beacon instance and actually target it though, it's not the POI target either. It shows as a white box (pic incoming)

Brief useless history lesson about this. When the game launched, Nav Beacons were functionally useless (they were originally supposed to be where you jumped into, early doors and that was removed). Players made threads sometimes complaining that they made no sense. Why do so many NPCs drop in and then jump out? But then, in one of these threads, one commander came up with the idea that the reason all these NPCs drop here first is to scan the beacon to get nav data for the system and that we should be able to do it as well.

Fdev saw the comment, replied and said "Great idea!" and it was implemented shortly after.

Always a pleasure :) Glad it's sorted. Just remember to save that data when you next dock 20ly away. I forget it all the time.
