How do inspection ratings work?

I always assumed an inspection that has 5 stars for all animals, and education, and cleanliness would be a 5-star inspection. But I just had one where everything checked out as 5 stars...but the inspecting rating itself was only 4?

Can anyone explain how they work, seeing as my assumption was obviously wrong?

Based on the responses here....I think no one knows. It's a bit frustrating not understanding and the help section in the game doesn't explain it at all. :/

I get it in sandbox mode too. Another thing that just started after the 1.1.1 patch is the Inspector spotting a protester every visit. I've yet to see one (since 1.1) and every animal habitat rates 5 stars. I don't understand it at all.
