It is pretty much straight forward, they ask for hardware info,
it can be found in the launcher, in options and display hardware survey,
copy it to clipboard and paste it in the support page and explain the problem you have.
You also need the version of the luncher,it can be found on the left side of the bottom
of the launcher, at the moment it is: 57605
