You need to check the mission boards for a specific mission to increase your rank. Each time you take a rank to 100% you need to find and complete one of these missions to actually get the rank increase.
They can be various kinds of mission (delivery, acquiring some commodity, killing someone) but you can identify them because they will make reference to being a Federal Navy mission in the title. They are given out by Federation minor factions just like other missions and you have a better chance of getting them from a faction with whom you are on good terms, so if you've been working a particular system or two to increase your reputation, I'd try there first.
The good news is that building rank continues in the background even if you haven't done the ranking up mission, so if you're at 100% midshipman now, as soon as you complete a ranking mission you'll find you're not at 0% for the next rank, assuming you've done some more missions after first hitting the 100% - you might already be 5%, 10% or more towards it depending on how many missions you did since hitting 100%.
You do need to be cordial, sometimes friendly, before you get such missions. If you've been all over the place, you might not be liked well enough to see such missions.
Find yourself a good fed system and get yourself allied, keep doing missions in that area and the promotion missions will come. I used 1 I Centauri, 5 fed factions and lots of fed factions in the surrounding area. Bear in mind your rep will stack, so when you do get a rank mission your next rank may start at between 1-10%.
I was friendly/allied with most of them, since I was Sirius permit hunting and gathering modular terminals in the surrounding area. Still nothing.
Hmm. I suggest a bit of board-hopping, then. Sometimes that helps.
As for the rank-up missions, yes, they were a bit of an issue after 2.4.01 I think, but that's been taken care of - I've managed a couple of Federak promotions myself.

This is THE only meaningful system in Galaxy where we (I at least) arrived trembling and full of hopes to see... It's....<beep>. :|

They are there, i was bestowed the title of 'Prince' from Duke, on Tuesday. found the mission to advance very easily as others have suggested, by looking in the Minor Faction missions i had been working my bottom off for.
