There are several threads about CMDRs who've killed their non-CMDR doppelgangers.

I haven't seen myself yet. I'm not sure if they pluck CMDR names randomly out of the barrel sometimes, or if they only use names submitted by kickstarters.

I think they've added to the list greatly over time, but AFAICT it's still entirely hand-authored.

I haven't seen "my" NPC for ages. :)

Probably, although the upcoming ship names might be procedurally generated.
Personally, I've yet to see the NPC I've named, nor heard about it from others. But then, so far the only NPC name I've ever seen twice was Chris Roberts.


And @ Chiggy: obviously there can be only one.
