Mistakes made that you lived through

My biggest muck-up is think it was a good idea to fly into a resource extraction site on the dark side of a planet, than get into a dog fight with my sidewinder vs. an anaconda. I was able to maneuver and put up a good battle but failed to remember that asteroids on the dark side are.. well.. dark. Smashed into one while thrusting laterally cracking the canopy. Life support kicked the bucket less than 250km to the starport upon retreat. :/
