I was just destroyed by an ally in a war zone. I didn't see any enemy on the screen and couldn't locate them on my radar but after it just said "you were killed by Eranin Defense fighter." There was a PC ally on screen at one point but i couldnt find them when i was being attacked and there was no commander name in the killed by box.

Eranin Defence Fighter is a NPC.
Chances are you accidentally shot him, probably while you were sighting an enemy. The NPCs have a nasty habit of getting close and personal to the enemy, regardless of whether you're currently shooting at them or not.
Always keep an eye on your scanner. If everything goes red, run.

I really wish you would just get a fine or a warning the first two or three times you hit a friendly. It's really not fun to have bits of you scattered across the system because of one stray laser. They really don't frack around either, they make it priority one to take you out and will fire all the missiles to do it.
