Car disqualified for running an unwanted extra lap in Q... W T F
This has happened to me, but my driver was not DQ'd. He was eliminated from qualifying because we ran out of fuel due to having to do an extra in lap.

I believe in my case it was caused by a driver in front of my driver blocking the pit lane entry at the end of our in lap.
It was at Miami, and the driver in front was continuing for another lap. My driver was on the wrong side of that car when we reached the pit lane entry, and because they are programmed to drive at a specific speed for in, flying and out laps, we were forced to continue on to another lap, for which we did not have enough fuel.
We ran out of fuel before reaching the pit lane again, and were forced to retire the car. As irl, you're not permitted to take any further part in qualifying if you can't make it back to the garage.

My solution? Concentrate on making sure my drivers aren't near any other drivers in qualifying. The safest bet is to try to be last over the line after the chequered flag falls. Do 2 runs in every Q session and expect that the first one might have to be abandoned because of traffic. If you are unfortunate enough to run out of fuel when you have to do an extra in lap... that's racing. Like a mechanical failure (which don't exist in the game, so count yourself lucky!) If you have a dominant car, you can do just 1 run and go out behind all the other cars. But you run the risk of your driver messing up and you'll be out of qualifying, of course.

TL;DR I think there are very few tracks where you might be unlucky and get blocked from entering the pit lane by another car, but you can definitely avoid it by being the last on track to set a time.
 
I used to try to be the last car out. But, because it's possible to have a critical lap ruined by one of the game's ridiculoulsy frequent incidents, I now try to be the first out before the cars undertake their final run in any given session (which itself runs the risk of leaving the pits too early and hitting out-lap traffic on the subsequent lap).
