Well, designers of war aircraft made similar mistakes in real life. There are accounts in WW2 of some aircraft just blowing up when the bomb release was triggered because the design was faulty. Of course they fixed it or got rid of the design pretty quickly. Even then when it worked you still have to time your bomb detonation timer right or risk blowing yourself up. Mistakes are still made today... We are only Human.
It was the remote controlled Lancasters that were not very safe. To say the least.
A dumb bomb is armed by centrifugal force, as are several RPG's and mortar grenades, this system is very, very safe.
The main grievances, were things leaking oil on windscreens, planes being insufferable hot, planes with no outlook, planes with too narrow undercarriage and planes that were just so underwhelming or generally poorly designed, that they were flying coffins.
