Whilst on a deep space Mining mission: The Type-9 Heavy, registered FL-01T 'LKS Yorktown' was lost with two crew members on-board, the captain was found unconscious in an escape pod, the second crew member was not found. No external factors are thought to have contributed.

Data from the black box revealed the ship left Omega prospect enroute to the HIP 17862 system on Fri 13 Apr 3304, 4:44pm, fuel load was 64 tonnes, enough for two days life support with all systems running. Toxicology reports confirmed the Captain was mildly inebriated at this point, but within legal limits. After 3 hours of unusually successful mining, the crew inexplicably decided to celebrate with a rare ancient earth brand of whiskey called Red Label. Voice recorder data confirms both crew members continued drinking and lost conciseness around 3am 14th Apr. Ship destruction has been confirmed to be on the 15th of April.

Safety Recommendations

Alcohol consumption to be moderated on all interstellar ships. Escape pods mandatory for all crew members. Ensure all crew members are trained to shut down non essential systems to reduce fuel consumption. Turn off ED when not in use..
 
My sincere commiserations on the loss of your crew member and also of your beloved Type 9, for nobody loves Type 9s as much as you.
It's the hardest part of being a Captain, letting the family know their loved one is gone forever.


We need to start a galactic petition to force ship manufacturers to equip crew escape pods.

"SAY NO TO ONE POD PER SHIP!"

p.s. If it's any consolation, my SLF pilot looks like an extra from "In the Flesh."
She's so disturbing, I've had to ask her to keep her helmet on at all times... Ostensibly for safety.

In related news, reports are coming in of an incident involving a Viper Mk3 near Marco Qwent's engineering facility on Lucifer.

It is believed that excessive speed, poor visibility and alcohol were causal factors in the collision with Qwent's antenna array.

One investigator at the scene of the crash commented by saying "There are no trivial collisions at 900 m/sec" before continuing to collect debris with a dustpan and brush.
 
Another Type-9 Heavy, the Spice Rhino was lost last week near Sol while delivering high value cargo. Long time treasured fighter NPC pilot Hayley Bean was lost in the encounter with an Anaconda. The pilot of the Type-9, Hans Mueller, made some critical errors after his canopy blew and thus the ship ran out of oxygen near the surface of a nearby moon.

Sadly, he panicked and ran when even though the Anaconda down to 2% hull and the Spice Rhino had 34% hull left. The Anaconda had recharged its shields and was inflicting heavy damage on the Rhino. When Mueller managed to flee he tried to land at a surface outpost as the nearest station was 65ls away. Trying to land on a station without the HUD proved too difficult and all hands perished when the O2 ran out.

There ought to be a feed on Galnet reporting Commanders' ships that have met untimely demises; should include reasons such as inebriated pilots, piloting with undue care and attention etc. Should be on the audio feed: this would be immersive.
I can just hear one report now: 'A pilot ran their vessel into an asteroid while listening to the weekly accident report.'

My commiserations 777, I've lost a high ranked pilot too. It's a bit of a pain! We should have escape capsules for our pilots too!
