Some missions give you a destination in unexplored sysytems, do I need...

...a surface scanner and discovery scanner, or can I just install discovery scanner to locate bases and starports?

I have limited room, just one bay free.

Your normal ship targeting scanner will do a scan of any body you target (range depending on body size) - this produces those "Level 2" scans you see in the stats page. Those scans are sufficient to produce a surface map I believe.
 
Unless my memory is failing all ports are indicated on the system map once you arrive, even without a scan. You will see broken lines and spaces for missing planets, but the ports are shown.

But I have been out in deep space for a while and might be going crazy - also not sure regarding planetside ports - do these now show up automatically like starports?

If you haven´t done system scan (honk) you can´t see settlements/star ports on planets in unexplored systems

No - surface installations only show after the body is scanned. You can tell they are there from the system map and from the extra "unexplored"s.
