Seriously, does mission availability really have a relation with reputation, or is it just some randomization?

The faction in case refers to the group offering the mission. Besides, they may like you enough to trust you with a shipment of coffee but not enough to deliver sensitive intel data. Make sure you're allied with the group offering the mission by either doing missions they allow you to or missions from groups affiliated with the one you want.

I get this also if I don't get to play for 4 - 5 days. I seems that you have to do a couple of missions for them and then everything is okay again. Kind of a pain, but I can live with it.

I think the problem is when you are Ally with the global Faction for example Empire you don't really see your Faction standing with a subfaction alligned to it.
