Here's what just happened to me, and I'm passing on a lesson learned. Since Ody release, I have completed hundreds of Ody on-foot missions and have never done the following series of events before, so this is a first for me. Today, I had completed several on-foot missions and was in the process of turning in the completed missions at a station terminal. At the time, I didn't realize it but I was close to the storage limit on my ody mats, and since I usually select the material mission reward, when I tried to turn in my last mission, I was unable to select the material reward because it would have taken me over the limit. No problem, right? So I zip around to the bartender and trade "up" a bunch of my low value ody mats to higher value ones and thus created ample storage space to receive the mat reward for my last mission. Then I zip back to the terminal, select completed missions, and it fails. Whisky Tango... Mike Foxtrot? Relogged, and did all the usual troubleshooting, to no avail. So I reviewed the mission from my transactions tab, and then it was obvious. The last completed mission was to salvage some electrical thingy from a crash site, and obviously I needed to turn this in for mission completion. But the BLEEPING bartender interface doesn't distinguish in any way that one of my material thingies, in a stack of similar electrical thingies, was a mission critical electrical thingy, so I realized that I apparently sold it without realizing this. So, I lost the 12 Manufacturing Instructions reward.
