I'll be on final approach to a station and going a bit fast with my finger on the J button waiting for the "safe to exit frame shift" and the "slow down" warning will appear instead, and I'll end up 1.2 Mm from the station. Or I'll enter a system and be looking for a location in my nav panel and find I've completely left the system by the time I've found it. Oh, the time I meant to throttle down and instead, shot stright downward and got stuck under the beams of the space station was a good one too.

Trying to repair my thrusters while in supercruise.
After a few hundred system jumps I dropped out of supercruise to go do something in RL (safest place to be) before scanning some planets. When I returned I went full throttle and just stared at the screen for quite some time before I noticed the remote planet insisted on reporting an arrival time of about 1.4 days.
For a brief period my brain just shut down dumbfounded about what was going on before a neuron fired and I hit my frame shift button.

I hate when I forget to request docking permission. Those trespass warnings can be annoying and scary. A module that adds a sticker to the windshield allowing you to enter without having to request docking permission would be nice.
I like sitting on the pads listening to the station announcer .. Dunno why .. just sounds cool.

Boosting out of the station entrance. 99% of the time, it's fine. It's the other 1% of the time that it's dumb.
^^This and launching and realising I'd forgotten to restock on limpets - so have to leave the station, turn round, remember to request docking again, and reenter and dock.
I have couple
- You know that gold found on SSS is trap but you still try to scoop it and then wonder why those 8 elite vultures killed me.
- You prepare to interdict assassination mission target in corvette and turn off something (no power to all modules) and you accidentally turn something on, no power and you get thrown out of super cruise and then the mission target enter your instance. Lets say sitting still in front of npc corvette with 2 plasma accelerators hurts a lot.

Either forgetting to request Docking permission or requesting it twice...mostly when drinking or when my mind is elsewhere.
I generally concur with the ideal that drinking alcohol is when the bad things tend to happen.
