What exactly does the option to reserve a commander name do? what format will my name be in? e.g. If I put John in the box, will my in game name appear as John? or will it be commander John? will it appear in any different way to names in other games, or should it just be a normal online game name like "xxkillerproXxX" (please forgive the awful CoD-esque example name).

A normal online name should do fine. We don't have precise details just yet, but my understanding is that you'll be something like "commander John". You might get a rank if you do naval missions ("captain John"). I think they've dropped the idea of epithets based on behaviour ("John the wise") but you get the idea.

It is the name of your avatar in game - so in my case it is Keeval and will appear in game as Commander Keeval (don't put the title in your name). It can be a single word or a proper name. The only restriction is that it cannot already be in use.
