I don't get into fights or battles so I haven't figured out what causes it. Have only seen it twice while playing.

PS. Why was the forum image uploader saying invalid file when I chose a 200kb jpg??

I've taken damage on various modules before and it hasn't appeared though. what is the threshold and how does it decide to display the icon?
