So most of the time, when I'm flying toward a station in supercruise, I'll be slowing down and watching the distance scale go from LS, to Mm and then, as soon as it gets to Km it's time to turn off my FSD. But sometimes I'll get the nod to disengage FSD when I'm a lot further out, like 10Mm or more. Is this a bug or am I accidentally doing something I haven't noticed?

Nope, not a bug. In some situations (like certain USS) the safe to disengage occurs much further away, in the Mm (million meters) stage of approach.
I have yet to work out which ones have a particular range, but stations are still always once you drop down below 1Mm (1000Km). At least that's been my experience.
As I understand it, the distance for a safe disengage is affected by large-mass bodies, so most stations (which are orbiting close to a planet) have the shorter disengage distance, whereas those that are farther from any large mass have a longer disengage distance. It's not a bug, anyway, but intended. It was added in one of the patches, and mentioned in the patch notes.
Yes that's correct. The distance/speed showing the blue zone (on the bottom-left of your main HUD) when you can safely disengage at a POI such as a station or USS will also increase from the standard 1.00Mm or 1.00Mm/s if the POI is further away from a celestial body/gravity well.
