Actually some times planets just are not there, this is very rare and has occured with me twice over two years. I had to restart the game and they appeared. But your answer could be the obvious one, though funny.

Maybe it is just cloaked?

Drop some cargo at the location it should be at and see where it moves. If you go into silent move at the same time, they think you blew up and might lower their cloaking field to launch fighters to go investigate.

The planet could just be out of range of your Discovery scanner.

I never go anywhere without an ADS, but if you have a basic or intermediate discovery scanner, you may need to circle around the primary star at 2x the range of your DS and re-honk repeatedly until it shows up.
If the system has a Nav Beacon, you can drop in there and scan it to get a full map of the system.
The GalMap may also give you the option to purchase map data for that system.

Best to just always have an ADS equipped though.

If you do have an ADS, and you used it, and the planet still doesn't show up on the System map, then there's some kind of bug or the planet doesn't exist.
Look for the target tourist beacon on your Navigation panel.

I have an intermediate discovery scanner,,, I have found 3 planets ( called 1,2, and 3 ) while the target body is supposed to be at planet 5. I have been flying off in different directions from the star for 100,000 ls or so, and keep using scanner, but nothing. Just hate to fly all this way and have to quit mission

And no,,, nothing on the scanner or system map other than the 3 planets and no nav beacon.
 
If there is no nav beacon go back to some station and replace scanner for advanced discovery scanner. If planet is far away from the main star you can never find it with less range on other scanners if you have no luck.

That's your problem, but there is a solution that doesn't require you to fly back home: use parallax to look for moving objects.

Go to the star, align your ship so that it is flying perpendicular to the plane of the ecliptic, and fly for a long distance; let's say fly until you are 20,000 ls away. Turn around and point your ship at the star. Fly towards it and look for moving dots against the background stars. When you spot one, fly towards it and honk your puny IDS as you approach. Sooner or later you'll get a hit.

(This is the method of finding planets that paid-up members of the "Nerf-the-ADS" brigade subscribe to. It is extremely tedious. Have fun.)
