Stick and FAon, KB/mouse and FAoff I would guess. At least I can't hit the barn with FAoff and stick.
I use full FA Off with an X-Box Controller and a mouse thrown on the floor with lateral thrust mapped to it.
I'm with you FAoff just doesn't quite do it for me flying with HOTAS. Mouse and keyboard? No problem there. Its weird though cause I can fly full Newtonian in Kerbal Space Program (no Mecjeb), orbit, rendezvous, dock, land, you name it but in Elite? Not a chance. Again though ... I don't use HOTAS for Kerbal.
I toggle fa-on when I need to aim rails with my joystick (unless I'm shooting a cutter or something like that). I do ok with aiming PAs with faoff, thats not much of an issue. Hitscan seems to be worse than lead weapons for me.
A lot of the full fa-off types use relative mouse so its a lot easier for them to aim then us joystick users.

Since I've mostly encountered smaller ships I've built a Cutter to overwhelm them and it performs quite well, especially against Chieftains.

Yes and yes. I believe in doing PP in open because that was the intention of PP. It is pretty fun to send medium ships cowering with their tails between their legs.
