Codex confirmation of different world

Greetings Commanders.
I have a question regarding codex entries.It may well have been answered before, so please feel free to point me in that direction! :)
I am out exploring The Elysian Shore at the moment and every so often, I get a codex entry pop-up.
When I go take a look, I find it is a confirmation, which is fine. However, the confirmation is of a similar world in a completely different system.
For instance: I was in HYPUAE AIM EZ-M B48-2, but i confirmed a world in SLEGOU XK-V D3-87 which is 3.7K Light years from me.
Anyone know why this is?

You should have two locations listed in the Codex for anything you discover:

Reported: the first location in the region that anyone found one of these
Confirmed: the first location in the region that you found one of these

They can be the same system but usually won't be.

I thought it a bit odd, but not complaining. I just wondered how it worked. Now I know.
So can there be more than one codex entry for any particular star or world type? or is it first find of each type only?

One per status per distinct region - so for example in Inner Scutum-Centaurus Arm, the "reported" B-class is Skaude JX-T e3-4, but my personal "confirmed" is Eol Prou IW-W e1-750 as the first one I scanned after the Codex was introduced ... and there are also various record-breaking ones listed from my personal finds, so for example the lightest is Eol Prou IW-W e1-2564 at 2.211049 solar masses (but other people have probably found lighter ones elsewhere)

Then in Tenebrae, the "reported" is Dryeou Flaae XY-S e3-0 ... and because I've never been there, no confirmed or personal bests.

In the first instance, as @Factabulous says, it can be regions. When you go to the codex on the right hand panel you see the map. It is showing the region you are in at the time of opening.

So if you find a black hole in one region, Codex tells you (the little pop up) one jump later you might be in the next region, find another black hole, you get another codex pop up.

The other thing to consider is:

it looks ‘completed’ because it’s blue but when you click into it:

You can see I’ve actually only discovered the Non-Teraformable, the orange teraformable is yet to be discovered. Maybe you’re finding different varieties of the same planet/star. This is also the same for organic structures.

You can also tell at just a glance if you have discovered all the variants...if the orange circle with the exclamation point is lit up and has a number next to it, it means there is something there that you haven't yet discovered
