How do discoveries in the Codex work?

I'm currently in my SRV in front of a lava spout, but my codex has only two entries for lava spouts, and these are both 'reported' by other players dating from 11th December.

I've been away from civilisation for a while so there's a good chance that that was the last time I docked at a station.

Why don't the multiple lava spouts that I have found come up as 'confirmed' in my codex? Do I need to dock somewhere? Do I need to select them somehow so that they are read by my SRV/ship?

Have you used the composition scanner on them? If not, that's probably your issue.

I have not. I had no idea that was even a thing. Thank you!

Now to figure out how to map it onto my Xbox controller so I don't have to keep grabbing the mouse for the scans.

EDIT: I've mapped SRV secondary fire onto the left shoulder button and moved Drive Assist to the Y button (you can't hold down the X, Y, B or A buttons without the game assuming you're going to use it as a modifier).
