Is F4 perhaps a system sensitive keybind? Y'know how on some computers that might be a shortcut to say a graphics card command or a screenshot or something. And just because I don't want to assume anything. Try it again now that you have that setting turned ON by binding to a completely different key, even just temporarily and HOLD the key down as the item wheel is a HOLD function and not a press.

Sorry if you have tried all this already, just double checking :)
yes I changed that to i, then when I load the game is back on left alt hold, obviously it doesn't save when I change it but even on left hand alt hold it still doesn't work
 
I really don't think you should be using Alt+(the key that you choose) if I understand you correctly. Try just using 'i' for instance or the number 4. The game doesn't seem to like more than 2 inputs. In Horizons you could hold 3 buttons and it would sort it out but I've noticed in Odyssey it only seems to recognize two buttons so if you are asking it to do Alt+(button)+(another input like left or up) it may be getting confused.

Try just using (4+up) for example. Or (4+mouse input)

Also it sounds like you are changing settings but then the game is reverting to older bindings which would then negate that menu context change that I pointed out. Make sure you go into your bindings and select 'custom' as this is the one that has your most recent changes in the control settings.

Maybe I wasn't clear.

To open the item wheel you need to bind it to one key only.
Then when you HOLD that one key and while HOLDING it pressed you can then press another key like 'up arrow' or 'left cursor arrow' to assign further directions in the wheel.
If everything is correct, just HOLDING the one key you have bound to Item wheel should make the item wheel actually appear and stay on screen for as long as you HOLD the button down.
