I don't know if this is the case. but if your treatment plant is breaking down it does decrease the efficiency of it. So just check to make sure mechanics are keeping it up to date!
Is it possible that it's too far away from any staff break rooms? This might explain why sometimes your mechanics have enough energy to make it far enough to repair, while other times they don't.
