Your first ever death? Mine happened just now.

Launched in a station, pointed myself at the mailslot, retracted my landing gear, set my speed for about 1/4 max speed. Hit shift-tab to reply to a Steam chat window. Tab is mapped to my afterburners.

Hit the mailslot doing ~350, exploded.

Read the manual, watched the vid's, and did the training exercises.
Jumped into my brand new sidewinder, yelled "Hi ho Silver, Away!"
Made 3 or 4 jumps to nowhere.
Upon not being able to jump anymore, realized that there was a fuel tank, it was empty, and despite google's results, there was, in fact, no such thing as a S.O.S. beacon for refueling.
Self Destruct.

Yesterday in a sidewinder killing stuff in a res and a jerk in a vulture rammed me , my wild fire hit someone insta wanted . Vulture then executed me . Came back with my vulture but he was not there . Can't wait for my revenge .

I was coming from RES sites with about 1 mil and a smiling face. Exit Sc ask landing permission and alt+tab for a moment. A friend asked me if I would play CSGO with him. I rejoin the game to see myself 10m from the space entrance cow cage as I hit that thing straight on. I bounced backwards and a Type9 killed me (npc) by ramming by when i was at about 15%
 
My first death was in the training mission to dock as I couldn't figure out the controls, my ship just kept spinning for 5 mins slowly towards the station until I pressed boost and finished myself off

Drunk flying during beta, hit boost inches away from the landing pad. Boom. Also got shot for forgetting to ask for docking permission and racing into the station. Still do this now, but just hit reverse and get back out!
 
I died on my very first day of playing, and than again twice when i was still in my sidewinder. After that, no death till yesterday, thats more than 3 months and i really started to think how good i am, LOL.

Yesterday after a night shift at work, and with good 30+ hours without sleep i missed the slot with my cobra, no boost, i just missed it and boom. I was like, ok, you are too tired for ranking up missions and landing every few minutes, so lets just do some RES bounty hunting. Well, after 20 minutes of fun i boosted my Viper in to an asteroid.............., after that, i just payed my insurence, and called it a day.
 
My first death? Was returning from a 14k ly trip with lots of exploration data on my old Cobra. I was 6k light years from civilization when my bro called me in the other room.
I go afk and after a few minutes I come back... My ship had 450%+ overheat, all systems were failing and hull was at 3%. I tried frenetically to recover but everything was useless.
I never understood what really happened (no suns nearby, no living things around etc), I think I lost around 20m exploration data and I learnt to NEVER GO AFK.

ouch... yeah i think it was silent running. How many hours of play time went down the drain =z ? Ive done a trip to barnards loop and VY canis majoris and that has taken me about 8 hours now. Im guessing you had no repair drones or reboot/repair options left? you can get back with 3% hull just fine as long as your frameshift drive, life support and thrusters are working.

Bounty hunting in a RES. My first clue that maybe it was a bad idea, was on the SC in, realizing the portion of ring I was heading towards was completely in the shadow of the planet- as in, the part that looks like it's not there. Dropped out anyway, managed to rack up a hundred thousand in bounties by being incredibly cautious. Got that last kill, decided to head out of the belt to cash in. Full throttle, hit the boost to clear the gravity well quicker, failed to notice I was heading towards a patch of black slightly blacker than the black around it. Boom today.

That was the last time I ever went bounty hunting in anything other than full sun RES's.
 
Just had my most expensive one by trying a 'Maverick' leaving the space port in my Python... 4.5m poorer and feeling I probably should stop trying to boost out of the mail slot ;)

Hunting an assassination target in my Eagle. Turned out to be an Anaconda with a plasma accelerator. Completely destroyed my thrusters, sending me into an uncontrolled spin. The funny thing was, after that he'd run out of ammo, and neither his lasers nor his missiles could get through my shields. Had to self-destruct, having no other option - but I still count it as a draw!

Friend of mine was trying out mining in his Sidewinder. He gets a message from a pirate looking to shake him down. Just as he's reading the message and the pirate starts firing on him, he accidentally grazes the boost button and boosts right into the asteroid. :D Pirate finished him off.
 
Funny how bad design makes some believe it's part of the difficulty, instead of something that needs to be fixed. Typical stupid humans.

First death by glitch. Second death by another glitch. 3rd death by the same problem the OP ran into. Haven't played since.
 
Mine was in the first couple of days playing in my stock sidewinder.
I drop out into an USS and there is an NPC Anaconda there. He's not moving and nothing on comms.
It was the first Anaconda I saw outside youtube videos and I remember thinking "Wow, cool ship, lets fly by and admire FD's work" when:
"- this was your last jump fool!"
Plasma accelerator BOOM no shields, no thrusters… beam laser BOOM

My heart was pounding and I thought to myself… this game is harsh I love it!
