They spawn in an additional NPC and they have multiple possible locations for the NPCs to stand at so you'd never get an open settlement.20 NPCs are standing a specific location leaving the settlement open for you to wander round and loot at your leisure.
You mean like FEDEX delivering multiple packages to multiple people in a large building? This happens thousands of times a day every day. This stupid mission design is done entirely to create an artificial grind. The core of this game is amazing, but the company behind it sucks.You're delivering to a specific NPC at a specific location. Once that slot is taken you can't get more as this takes the NPC off their normal route.

You may only take 1(One) mission to any settlement at a time. Once you've taken the first mission the destination is blocked. You can take multiple missions to multiple locations but they way they decided to handle missions is they load in a mission specific instance as you approach to prevent you noticing this as much they just don't let you have multiple missions.
