First off I made the custom resolution of 1280x768 @ 75hz w/GTF timing which can be found here https://forums.frontier.co.uk/showthread.php?t=215305 . Then set it as my desktop resolution. After setting all that up I simply ran the game (stand alone launcher, but I'm sure the steam launcher would work the same) and made sure the resolution/refresh rate matched the custom desktop resolution/refresh rate I made earlier. From there I'm sure you can adjust your graphics options to accommodate a smooth frame rate.

Previously the only way for me to get Horizons to work in VR (smoothly) was to use oculus runtime 0.5.0.1 in extended mode. I tried MANY things prior to this to get Horizons working as it should with oculus runtime 8.0 with no luck. It seems my specific issue, and possibly others as well, is related to the desktop resolution. Hopefully this will be addressed soon. (a little side note: 1280x768 sounds like it would yield a low quality image but the game "looked" just as sharp to me as 1920x1080 in 0.5.0.1 extended mode does. I think steamvr/ovr which seems to be the software that is allowing dk2 users to use direct mode in Horizons handles HMD resolutions differently than say a game that would directly access the dk2 through it's runtime)
Unfortunately even though this work around seems to be a step in the right direction there's still the issue of poor performance near or on planets, as well as in some asteroid belts. I feel like I get the same performance and possibly a slight increase (not by much) when using oculus runtime 0.5.0.1 in exteneded mode. I won't pretend like I know how their engine works, but I will say that when i'm in the middle of nowhere in my SRV, far from any settlements, that the frame rate is very poor. As soon as I look up or away from the planet surface and it's out of my FOV the frame rate skyrockets and all is smooth. THE only time I'm able to get a smooth frame rate is when i set everything to low and turn down the HMD quality ALL the way which seems like an unacceptable way to play.
My system is older, this I realize, but it's no slouch by any stretch. I'm running a I7 920 @ 3.8ghz, 780ti w/3gb of vram, and 12gb of system ram on windows 7 64bit with the latest nvidia drivers. I'm definitely looking into building a newer system made for VR, but I also hope that VR specific optimizations can be made to Horizons before then. I feel like it's within the realm of possibility, and appreciate all Frontier have done so far.
