His intention is to play the fool but in reality, he's playing the the crowd and eventually, he'll need to deliver.
The only way he'll achieve anything in relation to Scotland is with an English Parliament.
That can only lead to conflict between the Scots and English Parliaments as the two clash. Which will be inevitable if they are both equal. More so if the likes of Boris is running the English and Sturgeon the Scots.

But more, it will lead to another every expensive layer of government, the UK Parliament, plus the English, the Scots and eventually the Welsh.

Cameron's compromise, to prevent Scots MPs from voting on English matters, sensible, means that Scots MPs will be effectively prevented from taking many government posts. Mostly affecting the Labour Party, it will make Scots MPs second class.
