2. Mission-specific pirates are always the same one (till you kill them) and will attack you every time. (So not a bug.)
So - do you have a mission active?
Edit: There is another case in which a hit has been taken out on you because of abandoning or failing a previous mission. The NPC text for those goes something like "you will regret failing..." or similar.

When an NPC is chasing you everywhere, go to the station (not in Anarchy system). Wait just outside the no-fire zone for them to drop out and start shooting you, then boost towards the slot and let the police deal with them.

You can avoid this by never dropping while carrying cargo. If you must leave an instance after picking anything up, then go sell it. This way, the next time you drop in to an instance you'll be scanned, but not attacked, unless it's mission-related.
They tend to scan you soon after you drop, and then leave you alone if you don't have anything they can steal. So drop in clean, wait to be scanned and let them go find another mark, then get down to business in peace.

As I said, a mission-specific pirate or a bounty-hunter with a contract on you will always be the same one (name and ship) and will keep after you until you kill them.
