It's not difficult, you just position the freecam and then press Num-0, and it will follow you around while you walk. I just had to memorize the corridor layout so I wouldn't bump into walls. I do it all the time in 3rd person, I just use the mouse. my camera suite controls use my xbox controller, which is also used for srv/ship stuff. The controller is nice because the analog inputs allow your really smooth camera movenentI wonder if it's possible to turn the head while walking.
The problem here is that if you move the mouse in freecam mode, the entire avatar will turn, not just the head. With the keyboard shortcuts it might be possible to turn the head independently. Aren't you reattaching the camera after you position it? Than when you move your mouse you should only be moving your upper torso in the genera direction of where you're aiming, WASD still controlling movement. so if you do slight movement with no guns equipped I found you can get the head to look up and not the rest of the torso. I did like a little video with it because in the settlement power missions you can get some real spooky alien vibes going by just facing the camera at yourself and slowly moving characters eyes/ and face up to the out of frame ceiling, the neck even cranes naturally. I was wrong about the look left/right shortcuts, they don't exist. There's only look up/down. Yes, you can walk forward and strafe at the same time, but it looks a bit robotic. Anyway, this feature is great for machinima! You can keep the camera at a fixed position in the room, or keep its angle fixed while it moves around with you. Have you ever seen a karst landscape? Geological features like that aren't impossible or unrealistic. Not saying it doesn't need improvement, but it's definitely not unrealistic at face value.
