It's always touted as a synthetic opiate (and does have a decent Mu opiod receptor action) but also a great deal of action again serotinin reuptake and noradrenaline, hence the 'lovely' feelings. Structurally very similar to venlafaxine (the antidepressant) - hence what @Susanna says about dulling the pain but also 'not caring' about it...
tbh I absolutely needed them when I needed them.

'Funny' story. I was at the risk of running out over a four-day Bank Holiday, and phoned my GP for another script. The receptionist informed me that I would have to come in to the surgery and be seen by a doctor to get some more. I said I was immobile, told her why, and 'politely asked' her to check. About twenty minutes later somebody else phoned me and asked where was best to send the drugs.

Later my GP asked me about that time. I told him that I was in so much pain I couldn't see, even though my eyes were open and was completely immobilised by the pain when it came. Completely. Couldn't stand, couldn't even try. He nodded, patted me on the knee and said (to the effect) 'Yes Mr PiLhEaD, that's real pain.'

I think I should explain 'PiLhEaD' - it's not at all what you might think, but the name I sometimes used when dabbling in music production/engineering/remixes/etc etc... don't bother looking... it never went anywhere. I still have some tracks on soundcloud - pretty much all are just rough sketches for sharing ideas with [various] so please don't judge my mixing skills on these.

Seems you have had your share of real pain too!
I've been there as well. Believe me I could tell a story or too. But I'll leave it at peri-anal hematoma excised with a scalpel...

My daily regime...

3AM 2x50mg Tramadol capsules (I usually wake up from the pain)
~8AM 2x50mg Tramadol capsules (so I can get out of bed)
2PM 2x50mg Tramadol capsules (top up dose)
8PM 2x50mg Tramadol capsules (enough to get me asleep at around 10:30)

Some days I've had to 'double dose', that's fun when you are at work!

Edit : yes I did drive home after work... 😵‍💫
Wow - sorry to hear that - I was a very low-functing zombie on Tramadol. Dribbling was an effort.
