Phase warp travel is still experimental and can have side effects... please consult your travel agent. (yeah, happens to me sometimes too... was a known issue in Alpha, and is apparently back to haunt us... relogging sometimes, fixes it... sometimes having to exit the game entirely and then log in fixes it... but it is kind of a random event)
