As for fears, while exploring strange phenomenon, your shields go down and the computer warns you of unauthorized use of an airlock. You start hearing ...sounds. Of something making its way through the ship. But you're of course still locked into your chair (Spacelegs, where art thou?), and completely helpless against whatever has boarded your ship and is approaching the pilot's chair from behind...
What is the most terrifiying thing you can imagine while exploring?

Losing power and being trapped in a confined space in the dark waiting for the air to run out (probably stems from an amalgamation of my worst diving experiences).

FSD malfunction while mid jump, unable to return to normal space.
but when you would finally exit, after one eternity later, you'd be somewhere in the middle of Andromeda Galaxy. Imagine... ENTIRE galaxy. Unexplored...
Realizing I have to spend again hours of my life to grind materials that are empty and I want to pimp a module or weapon.

but would like to see more risks like...
warping on to a system with an unstable start... spewing out really big CMEs, so much so it's extra hot and unstable flying
or
Magnetars that interfered with your ships scanners, damage and shields you have on board.. malfunctions heat-sinks
or
Warping in on a large debris field, a plante that has been ripped apart near a start... having to avoid the chunks else you drop out of super cruise
and
make black holes a risk... if you drop out in normal space, you get pulled... hull damage as you strech, and have to try and jump out of there before you fall in... that may be to much for ED

I fall a sleep because I get bored fast, hit with the forehead in to the keyboard and go unconscious. Alien hidden on the roof kidnap mi and do bad things from behind and leave wasted on planet with depleted resources...

Being able to explore cave systems, in the knowledge that cave systems are not always uninhabited. And knowing things that inhabit caves are never happy for their sleep to be disturbed...
Or cracking a high-value asteroid, when suddenly, in the plume of smoke, something else stirs. You have disturbed a nest of voracious metal-eating space worms that bursts out and swarms towards your ship. HULL BREACH DETECTED. TAKING INTERNAL DAMAGE.
Or the spooky comedy cemetery that is most of this thread.
Or cracking a high-value asteroid, when suddenly, in the plume of smoke, something else stirs. You have disturbed a nest of voracious metal-eating space worms that bursts out and swarms towards your ship. HULL BREACH DETECTED. TAKING INTERNAL DAMAGE.
