I've had 2 insurance claims, totalling about 100,000cr or so, playing Open since 2013. My first was in 2017 and my second was just a few weeks later when someone griefed me in Shinrarta Dezhra. I basically gave up playing after that point, and it took me a long time to try going back. I play Solo now.
 
Not as many as some, more than others. Less than 100. Usually my fault for Super-Snoozing. Once was by Thargoid for screwing around too much, a couple by NPC either due to way OP engineering or being too new.

Most of mine were suicidewinder clear-my-name or I-just-don’t-want-to-fly-all-the-way-back-don’t-rebuy trips.

Been blown up three times by other players, twice by request to clear bounties and give them some credits, once by some who inserted himself into a non-PvP PG - I was in a cheap ship and didn’t even bother trying to evade, just reported to the PG ops for removal.
 
Playing mostly as a non-combatant, I've only had 11 insurance claims - and a fair percentage of those happened when I first started playing, as a result of my ship on several occasions deciding to ram itself repeatedly into the wall of a station until it died.

Eventually I figured out what was happening: at that time I had left and right thrusters bound to the foot pedal axes of my rudder, and the left switch would stick for a while in the open position. I changed my rudder and my thrusters binding and the problem went away
 
More than several!

Funniest one was when I slammed a brand new Asp Explorer into the station wall on the very first time I took off with it, and went straight back to sidewinder as I didn't have enough credits to pay for insurance. This was in the game's early days, when you still needed to play for almost a month to get enough credits to buy a mostly E-rated Asp Explorer, so it sure teached me a lesson :D

Additionally, you can add the most memorable destruction if there's one. Mine was in a CZ where I went with my then-brand-new Corvette, all confident that it's essentially indestructible. Turned out it wasn't... the memorable part was that I saw it coming, the moment my shields were gone. I had too much trust in my armor only and by the time I decided to bail out I knew it was too late. It was the most painful "death" due to the sheer cost of the ship. Since then I engineered the s*** out of it just to be sure. :)

Thirty two rebuys since Jan 2018. Some early mistakes in Solo, some exploration mistakes (dozing off) and some Open combat leading to getting thumped by other Cmdrs.

However my last one was getting taken down by an Elite NPC Krait MKII when I intiated a fight with my last Frag cannon shots, I hadn’t realised, then decided to finish the fight by ramming him in my FdL - he was down to 8% hull and I was at 54% when I caught him with a cracking boost ram and sparks and fire leapt toward me. The smile left my face when it dawned on me that the flames and explosions weren’t coming from his ship but mine.

I think he must’ve cheated and varnished his Krait like you did with your conkers. ;)
 
Playing since early beta. Last death was during the beta as well.

0 claims since the account reset at the end of the beta.

I'm not a God tier pilot or anything, I'm just super careful (RL commercial pilot habits die hard) and fly the fastest ships money can buy.
 
Without logging in I'd guess over 100 rebuys. Most memorable was once when I got tired of waiting in line to get out of the slot in a space station and decided to attempt it manually and got destroyed. I have also forgotten to get permission to dock and accidentally fired at the inside of a space station.

116 insurance claims in ~4 months...that's <ahem> really bad!

Actually, at least 2 were definitely due to catastrophic bugs. So only 114 then...phew, that's much better!

Mitigating circumstances in my defence: I've never used a docking computer or gimballed/turreted weapons & I discarded my shield after my first week or so. Hence, as you can imagine there were a disproportionate number of deaths in my first couple of weeks.

Latterly, before I engineered my ship - specifically my weps - I regularly ran out of ammo on my first HazRes target therefore I'd just self-destruct because I had no bounties to hand in & I've also taken on wings of high ranked, high powered ships in my unengineered, shieldless DBS from my earliest combat days with a view to "learning to swim" by diving in at the deep end...hmmm on reflection, I wouldn't necessarily recommend this particular strategy.

Yeah-yeah, I'm still crap...I know!
 
This was about three months ago (I'm at work). It's probably about 220 by now, I've been hanging around Jameson a lot 😳

3 definite suicides.

A couple of heavy landings on planets.

One terrible experiment with a brand new FDL where I just cooked myself during combat (sold it straight after the rebuy).

One by a human CMDR in my one and only foray into OPEN (10 mins, start to finish).

One docking incident that saw me reverse into a couple of different ships, I was trying to land my first ever medium sized ship manually.

One smuggling shambles where I tried to speed through the mail slot and couldn’t work out why my brakes weren’t working (I was holding the scanner button).

The rest are too shameful to speak of.
 
Six rebuy screens, not counting SRV/SLF shenanigans. Actually I think the last one was my one and only PvP-related death, about 2 years back.

I’m an SAO fan, so I take dying in a game like this a bit more seriously than most. I won’t sacrifice a ship for lolz, or for “experiments”. I’m also not afraid of running away.
